Triage Support 
Providing guidance when patients need it most.

Ms. Grace Smith (66) called the 24/7 clinical support line because severe dizziness, numbness in her fingers, a sharp pain radiating from her neck down her leg, and chest pain. The patient care coordinator transferred Ms. Smith to an RN, who determined that Ms. Smith needed to be seen by a healthcare provider and booked a same-day appointment. The RN called Ms. Smith the next day to check on her. She was feeling better after her provider visit, and the RN answered her questions about her newly prescribed antibiotics. Ms. Smith said she would be reaching out to her father’s provider to get him enrolled in ChartSpan too! 

Emergency Intervention 
Saving lives through timely intervention.

After experiencing chest pain for three days, Richard Johnson (79) called ChartSpan’s 24/7 clinical support line, reporting chest tightness, difficulty breathing, a dry cough, nausea, and pain rated 6–7. Despite his symptoms, he was reluctant to seek care, feeling he couldn’t leave his shop.

Recognizing the urgency, the care coordinator escalated the call to a triage RN. Using Schmitt-Thompson protocols, the RN identified emergent symptoms and urged the patient to call for an ambulance. EMS arrived promptly and transported him to the hospital.

The next day, the RN followed up and learned the patient had been admitted to the ICU with kidney failure and abnormal potassium and blood sugar levels. Grateful and recovering, the patient said ChartSpan “saved his life” by encouraging him to seek emergency care.
 

Safety & Crisis Response 

Protecting vulnerable patients. 

ChartSpan supported a bed-bound patient, Ms. Mary Jones (72), in a distressing situation. After an argument, her daughter—also her primary caregiver—left for the day, leaving Ms. Jones alone and unable to care for herself. Bed-bound with a catheter since a car accident a year earlier, she shared feelings of neglect and isolation and had not yet informed her provider.

Recognizing the urgency, a mental health nurse joined the call and used Schmitt-Thompson protocols to assess for elder neglect. The nurse promptly reported the case to Adult Protective Services and notified the patient's provider for follow-up care. ChartSpan also connected her with essential services, including transportation, meal delivery, and home health support.

Since then, Ms. Jones has regularly reached out to ChartSpan, calling us her consistent source of help. In a recent conversation, she expressed deep gratitude, saying it brings her comfort to know there’s always a nurse she can rely on. 
 

Appointment Assistance

At one clinic, a patient was directed to a kiosk to schedule an appointment. She found the process impersonal and called ChartSpan’s 24/7 Care Support line to request appointment support. We quickly scheduled her visit—an experience she described as “bang/boom.” She said she would recommend ChartSpan to others with a "10+++."
